How Obscyro maps SNOMED CT to ICD-10.

ICD-10 mappings come from SNOMED's official extended map refset. Each mapping carries a mapGroup, mapPriority, optional mapAdvice, and (when present) a mapRule that Obscyro parses into a structured conditional (IFA clause or raw expression).
